Scanner document feeders
Document feeders detect page edges automatically. If a page goes through slightly off-axis, it can be rotated 90° or 180° — often inconsistently across a multi-page scan.
Phone-scanned documents
Phone scan apps often produce pages rotated 90° because the phone was held in landscape mode. Common with Adobe Scan, CamScanner, and similar apps.
